For this assessment criteria, you will need to demonstrate to your assessor that you apply person-centred values in your day-to-day work, taking into account each individual’s unique history, preferences, wishes and needs.

In the previous section, we looked at how to establish an individual’s history, preferences wishes and needs and how this information will be documented in their care plan. Therefore, you should be familiar with the care plans of all the individual’s that you support as well as having a positive working relationship with them, which includes trust and communication.

By implementing an individual’s care provisions in line with their care plan, you will be working in a person-centred way.
