If unsafe practices in care settings are observed then they should be challenged immediately to prevent harm from occurring and protect the welfare of the individuals that you care for.

Unsafe practices should also be documented according to your organisation’s agreed ways of working and reported to a designated person, usually your manager.

When you raise and report concerns, you have a duty of care to ensure that they are responded to correctly. If your concerns are not taken seriously, then you should follow it up.
